Transthoracic Approach of the Adult Morgagni Hernias

Abstract

Two types congenital diaphragmatic hernias are recognized. Morgagni hernias have been reported to occur in up to %5 of patients described in large series. Occasionally these hernias remain asymptomatic and are not discovered until the patient reaches adulthood. A 18-year-old female was admitted to investigate abdominal pain, fatigue and retrosternal pain. Breath sounds were absent on the right inferior region. A right-sided Morgagni hernias was detected on chest x-ray. Hernia sac was attached to pericardium on chest CT. The hernial sac was excised and the diaphragmatic defect was repaired by left thoracotomy. Morgagni hernias most easily repaired through an abdominal approach. We prefer thoracic approach because the sac was adherent to the pericar- dium.
